Session Fees:

Please note all sessions will be submitted to medicare for payment, with possible gap payment of up to $80 as required.

Your account must be paid in full prior to requesting any further services.

Medical report fees:

$150 per page (number of pages depend on the complexity of the report).

These fees are not covered by Medicare.

Other fees:

• Prescriptions request (non-urgent): $30.00
• Prescriptions request (same day service): $50.00
• Filling out forms/writing a note: $30.00
• Telephone consultation: $30 per 5 minute block (first 5 minutes free)

These fees are not covered by medicare. However, you can avoid these fees by requesting these service during your consultation.

Non attended session fees:

I require 2 full working days (Monday-Friday only) notice should you need to cancel or re-schedule your appointment. You must do this by telephone.

• If you give us 2 or more business days’ notice: no fee
• Less than 2 business days’ notice: $100.00
• Less than 24hrs notice: $200.00

This fee must be paid in full prior to requesting any further services.

Out of courtesy to our professional relationship, I request that you provide at least 7 days notice to cancel an appointment as I have limited appointments available.
